Immediate Implant Placement with Simultaneous Regenerative Procedure in Fresh Extraction Socket

چکیده
Aim: The aim of this case report was to reconstruct the alveolar buccal bone plate lost in consequence of a root fracture in the maxillary central incisor area after immediate implant placement (IIP). A 48-year-old patient was referred to our office with the chief complaint of spontaneous bleeding in his left central incisor. After careful examination, the following comprehensive treatment approach was carried out: (1) Atraumatic tooth extraction, (2) IIP, (3) provisional prosthesis installation, (4) alveolar buccal bone plate reconstruction with autologous bone collected from the maxillary tuberosity, (5) gingival augmentation procedure with connective tissue graft removed from the palate, and finally (6) adjustment of the provisional restoration. After 1-year postoperative, an adequate esthetic outcome was achieved with lower cost, reduced morbidity to the patient, and lower treatment time. This approach was able to improve gingival architecture and bone volume with adequate thickness and width, enhancing patient esthetics and satisfaction. In conclusion, when proper diagnosis, precise surgical techniques, and appropriate multidisciplinary approach are employed, IIP followed by immediate dentoalveolar restoration might be considered a safe procedure to restore the peri-implant bone and gingival recession favoring the achievement of appropriate esthetic outcomes. Clinical significance: The treatment planning employed was able to improve gingival architecture and bone volume with sufficient width and thickness enhancing patient esthetics and satisfaction in one single-stage procedure.

Sinus floor elevation via the maxillary premolar extraction socket with immediate implant placement: a case series.
BACKGROUND When immediate implant placement is considered for teeth with close proximity to the sinus floor, apical extension of the osteotomy is significantly limited, and often a staged approach is used. Implant placement into fresh extraction sockets and sinus floor manipulation using bone-added osteotome sinus floor elevation with implant placement are techniques most often used independent...
متن کاملImmediate Implantation in Maxillary Molar Sites: A Literature Review
Context: For many years, the standard protocol for implant placement was the delayed method which is to wait until the bone is matured after tooth extraction. Evidence Acquisition: The increasing demand for shorter rehabilitation times have led to the introduction of immediate implant placement. Although this method offers many advantages like minimal number of surgical procedures, less morbidi...
متن کاملHuman histologic verification of osseointegration of an immediate implant placed into a fresh extraction socket with excessive gap distance without primary flap closure, graft, or membrane: a case report.
The aim of this research was to verify clinically and histologically whether an excessively large horizontal and vertical gap distance of an implant placed into an immediate extraction socket would osseointegrate coronally at the implant-socket interface without primary flap closure, a bone graft, or a barrier membrane. An immediate implant and straight-profile healing abutment were placed at t...

Dental implants were initiated in 1922 by Branemark, who and associates described the relationship between titanium implant and bone which termed osteointegration, defined as the direct structural and functional connection between living bone and the surface of an implant (Albrektsson et al., 1981). Immediate implant placement, defined as the placement of dental implant immediately into fresh e...
متن کاملAn innovative approach for atraumatic tooth extraction for immediate implant placement by using Sapian extraction kit: A case report
Immediate placement of dental implant into the fresh extraction socket has been an acceptable procedure for the past two decades. Commonly, immediate placement has been reserved for the single rooted anterior tooth and single or bi-rooted premolar tooth. To preserve the alveolar bone proper, particularly that of the labial and lingual plates of bone it is essential that the extraction should be...
